Additional information for incoming 6th graders:
1. Tdap BOOSTER REQUIRED FOR 6TH GRADE STUDENTS
The 2006 Virginia General Assembly passed a law which requires all 6th grade students to have a tetanus, diphtheria pertussis (Tdap) booster shot no later than the start of the new school year. Please review your child’s immunization record. If your child has not had a recent Tdap vaccine he/she will need it to start in 6th grade. Please call your doctor or the health department if you have questions. Shots may be obtained from your doctor, military clinics, or the health department. Documentation should be taken to your child’s middle school if your child receives the booster shot in the summer. Your child may not attend middle school on the first day of school if they have not received the shot or if the middle school does not have documentation of the shot. If your 5th grader receives the shot during this current school year, please provide written documentation to the elementary school. This will ensure a smooth transition to middle school. If your doctor will not administer the Tdap at this time, documentation from your physician is required before entry into middle school.
2. Athletics- 6th graders are eligible to participate in cross country, wrestling, and track. Prior to the first practice/tryouts of any sport, a completed VHSL physical form needs to be turned in. Athletic physicals must be current, having been completed after May 1 of the current year. An athletic physical will be effective through June 30 of that academic year

Custody Papers (If student does not reside with a natural parent, legal custody papers from the court are required for enrollment.)
Proof of Residency (see attached sheet for requirements)
Transcript from previous school(s)

Copy of IEP or Special Education Records (if applicable)
