Middle School sports requirements

• Students need a complete, current VHSL physical dated no earlier than May 1, 2023

• All four pages of the physical form need to be completed. There are multiple places for signatures, please double check before you turn it in!

• Physical forms are available HERE or at the Montevideo office.

• Valley Urgent Care is offering $15 sports physicals.

• Physicals must be turned in before students are able to participate in practice.

• 8th graders who worked out at Spotswood High School over the summer need to bring a copy of their physicals to Montevideo in order to be eligible to participate here.

VHSL Physical Guidelines

Students interested in participating in middle school athletics are required to have a VHSL physical on file. The date of the physical must be after May 1, 2022. You may print a copy by clicking the maroon button above, or you may pick up one from the main office.

What to do if your child does not have insurance and wants to participate in athletics:

K12 Special Markets is the voluntary student accident insurance for those parents/guardians who would like to purchase accident insurance for their children. Insurance is mandatory for students participating in athletics.
